Refer & contact

Referrals, minus the runaround.

One phone call or one form. Matt reads every referral personally and comes back to you — usually the same business day.

Pathways

However they're funded, there's a way in.

Support at Home

Care managers & providers, GPs, family members, self-referral.

Helpful to include

  • Client details and Support at Home classification
  • Current concerns — falls, function, equipment, home safety
  • Relevant medical history or recent hospital stays

We coordinate assessment scheduling and reporting with your care partner or provider.

NDIS — self & plan managed

Participants, families, support coordinators, plan managers.

Helpful to include

  • Plan goals and plan dates (reviews drive timelines)
  • Previous OT or allied-health reports, if any
  • Plan manager's invoicing details, if plan-managed

We see self-managed and plan-managed participants. We do not currently see NDIA-managed participants.

Private

Anyone — no referral needed. GPs and other health professionals welcome.

Helpful to include

  • What prompted the call — a fall, a diagnosis, a worry
  • Home situation — living alone, stairs, bathroom layout
  • Any deadlines — discharge dates, family visiting

Fees are quoted in writing before we begin. Private health rebates may apply — check your fund.

Telehealth

The same thoroughness, through a screen.

Telehealth isn't a compromise — for the right tasks it's a genuinely good tool. Interviews, goal setting, carer training, report reviews, equipment follow-ups and many assessments work beautifully by video.

  • What you need: a phone, tablet or laptop, and somewhere quiet to sit. We send one link — no apps, no accounts.
  • What it suits: consultations, reviews, education, carer training, many assessments.
  • What still needs a visit: physical measurement, home modification scoping, complex seating. We'll always tell you honestly which you need.

Common questions

Asked often, answered honestly.

Not currently. We work with self-managed and plan-managed NDIS participants — this keeps our admin light and our time where it belongs: with clients. If you're NDIA-managed, we're happy to point you toward registered providers in the region.

Travel within Mount Gambier is included. For longer runs across the South East we charge travel transparently, quoted up front in writing before any visit — you'll never see a surprise on an invoice.

Typically one to two home visits plus report writing — around two to four weeks from first visit to final report, depending on complexity and funding deadlines. Tell us your plan-review date and we'll work backwards from it.

Gladly. We can work with your preferred local builder or recommend trades we've worked with before. Either way, we provide the specification, check the work against it, and sign off only when it's right.

For many tasks, yes — interviews, education, reviews and some assessments translate well to video. For anything involving physical measurement or complex equipment, we'll recommend a home visit instead. We match the method to the job, not the other way around.
