Why home

Good therapy doesn't happen in a clinic. It happens in the hallway where you turn with your walker, the bathroom that needs to work harder for you, the kitchen where you still want to cook.

That's why we come to you — specialist occupational therapy, delivered where life actually happens.

For older Australians with a Support at Home classification. We work within your funding to keep you safe, mobile and independent — from equipment and home modifications to reablement-focused capacity building.

We liaise directly with your Support at Home provider and care manager, so nothing falls through the cracks.

What we do

Seven ways we help you stay capable.

Every service starts the same way: with you, in your home, looking at real life. Then we build the plan — assessment, equipment, modification or therapy — around your goals and your funding.

Start a referral
  1. Functional capacity assessments

A complete picture of how you live — what you do, what you want to do, and what's getting in the way. Comprehensive FCA reports for NDIS plan reviews, Support at Home and other funding applications.

NDIS reviews · Support at Home · Funding evidence

  2. Home modifications — minor to major

From grab rails and ramps to full bathroom redesigns. We assess, specify and work alongside builders and funding bodies to get modifications right the first time — measure-ups, drawings and specifications, builder liaison, funding justification.

Grab rails · Ramps · Bathroom redesigns · Builder liaison

  3. Assistive technology — minor to complex

The right equipment, properly prescribed. From daily living aids to complex seating, power mobility and custom scripting — trialled with you, justified to funders, and fitted to your life.

Daily living aids · Seating · Power mobility · Scripting

  4. Pressure injury management

Prevention first. Seating and positioning assessments, pressure-care equipment, and skin-protection programs designed around your routine — working with your nursing and medical team.

Seating assessments · Pressure care · Positioning programs

  5. Falls assessment

A room-by-room look at your falls risk — balance, vision, footwear, hazards, medication-review prompts — with practical changes that people actually keep using.

Home hazards · Balance · Prevention plans

  6. Home assessment

The foundation service: a thorough audit of safety, access and function across your whole home, leaving you with clear, prioritised recommendations — not a report that gathers dust.

Safety audit · Access · Prioritised recommendations

  7. Capacity building

Therapy that builds you up: strength, balance, routines and skills for daily life, community access, and the goals that matter to you — reviewed and progressed as you do.

Daily living skills · Community access · Goal-focused therapy

How it works

Simple from the first phone call.

  1. Say hello

Call or send a referral. Matt will have a short, human conversation with you about what's needed and whether we're the right fit.

  2. We come to you

A face-to-face home visit at a time that suits — or telehealth if you prefer. No travel to a clinic, no waiting rooms.

  3. Assess & plan

A thorough assessment, clear goals, and honest advice about what your funding can cover before any work begins.

  4. Report & deliver

Reports, equipment and modifications arranged and followed up. We stay involved until it's working in real life.

Where we work

Based on the coast. At home everywhere on it.

Mount Gambier · Naracoorte · Millicent · Penola · Robe · Kingston SE · Beachport · Port MacDonnell · Bordertown · Keith · Lucindale · Casterton — and everywhere in between.

Further afield? Telehealth brings the same thorough assessment to anywhere in South Australia and beyond — all you need is a phone, tablet or laptop.

How telehealth works

About

Clinician-owned. Deliberately local.

Coastwise is Matt's practice — a clinician-owned occupational therapy service built for the Limestone Coast. When you call, you speak with Matt. When we visit, it's Matt at your door. And the report that lands with your funder is written by the same person who sat at your kitchen table.

Being mobile means we see the real picture — the step at the back door, the chair that's too low, the routine that matters to you. Face to face across the region, or by telehealth when that suits better. That's where good occupational therapy starts.
